The Employees State Insurance Corporation (ESIC) on December 04, 2023, invites applications from Hospitals, Diagnostic Centers and Blood Banks that are empanelled with CGHS, for providing Super-Speciality Services to ESIC Beneficiaries of ESIC Hospitals of Delhi and NCR for empanelment for two years for providing Super Speciality Services (tertiary care only) as per clause 14.7 of page no. 21 of referral policy.
The interested HCOs may submit their application up to December 31, 2023, as per Annexure A1, A2, A3 and A4 whichever is applicable duly filled along with the required documents mentioned in the application form on pages no. from 96 to 121 of the referral policy.

The Applications with documents should be submitted in physical form at a special counter for SST Empanelment, Med Branch-II, 5th Floor, D(M)D Office, Admin Block, ESIC Hospital Complex, Basaidarapur, New Delhi-110015.
