The Ministry of Health & Family Welfare (MoH&FW) on January 17, 2024, issued the Policy on Transition of Care (ToC) for Youth with Special Health Care Needs (YSHCN).
The following has been stated:
• Goal of ToC:
The American Academy of Pediatrics states, “The goal of transition in health care for YSHCN* is to maximize lifelong functioning and potential through the provision of high-quality, developmentally appropriate health care services that continue uninterrupted as the individual moves from adolescence to adulthood”
With continued good quality health care, the young adult is envisaged to attain full developmental potential and will be a productive member of the society.
